Representative case of open lower limb traumatic injury. A A 63-year-old man who was injured by mine injury. Tibiofibular fracture was noted and initial wound has no open wound. But the injuried site was necrotized gradullay then the knee joint exposured with tendon and muscle exposure. B Radical debridement was done and skin graft was proceeded to cover the defect. The exposured joint was covered with microsurgical free tissue transfer using an anterolateral thigh flap. C The patient can escape from amputation and can achieved full ambulatory status at 8 months after the operation
